Ideally, harm reduction is grounded in social justice with its emphasis on equity, and aim to minimize drug-related harm. This study examines Swedish drug policy, in which the social justice perspective has historically been less dominant. Harm reduction is not formally recognized as an independent part of Sweden's drug policy, and it often competes for resources with control, prevention, and treatment. However, in 2024, the Drug Commission of Inquiry proposed revising the aim of drug policy – from achieving a drug-free society to a society with reduced harm from drugs. A key focus of this study is stakeholders and their potential influence on Swedish drug policymaking. By analyzing key informant interviews (before the inquiry) and stakeholders' referrals after the final report by the Drug Commission of Inquiry, we aim to answer the following research questions: What is the significance of commissions of inquiry in drug policymaking? What arguments supported the stakeholders’ initial positions regarding harm reduction, and if and how were these positions articulated in the official written referrals following the inquiry? The Policy Constellations Approach serves as a methodological framework for this study. It will illuminate the distribution of stakeholders' power positions and interests in developing national drug policy. This paper contributes to the discourse on social justice and drug policy by analysing stakeholders' perspectives on the development of harm reduction initiatives, and the debate surrounding the proposal. It illustrates the efforts of some stakeholders to enhance the efficacy of such policies, while others attempt to impede such policy shift.
